What is the RCF Resident Care Manager (RCM) experience?
As the RCF Resident Care Manager (RCM) you will provide oversight and direction for the resident health care functions to ensure high quality resident care and a positive working environment for assigned areas of service. In this capacity, you'll be expected to:
- Assess residents' needs, coordinate care plans, and foster positive relationships with residents, families, and staff members.
- Conduct thorough evaluations of residents' physical and mental health, family & community resources, and the physical environment.
- Ensure that pre-placement screenings are completed to assess the appropriateness of placement and readiness to meet resident care needs prior to admission.
- Collaborate with interdisciplinary teams as you develop, implement, review, and revise individualized care plans/service plans for each resident to ensure assessments are conducted regularly and whenever there is a change in condition.
- Oversee resident medication administration, maintaining accurate medical records, and delegating nursing tasks appropriately.
- Communicate with physicians to plan care for residents as needed and coordinate services with outside medical service providers.
- Build strong relationships with residents, families, staff members, and supporting functional groups through proactive communication and feedback.
- Other duties as assigned.
What are the job qualifications?
- Registered Nurse in the state of Oregon, BSN preferred.
- Minimum 2 years nursing experience, long term care setting preferred.
- 1-year supervisory experience.
- Able to interact well with staff, residents, and families.
- Strong time management, organizational, and project management skills.
- Must consent to and pass a criminal background check.

- Pay range: $45-50.00 p/h, depending on experience.
