This position within the Terwilliger Plaza Community offers opportunities to serve our independent members in our Plaza Health Clinic as well as providing RN services and support to our licensed In-Home Care Agency, Ally Services (also under the umbrella of the TP community of care and services). The Plaza Heath Clinic is funded largely by our foundation to provide basic and non-urgent care-related services, in coordination with a member’s health care team. As the RN for Ally In-Home Care Services, you will provide nursing treatments, evaluations, assessment and monitoring as required by OH, including participation in the development and monitoring of client service plans, clinical documentation, and communication with physicians and pharmacists. Compliance with OSBN standards and with the OAR’s governing On-Home Care agencies.

Essential Job Functions:

• Sees Terwilliger Plaza members in Plaza Health Clinic, evaluates and assesses needs and provides needed treatments and interventions as allowed under the RN license on a fee for service basis in coordination with members health care team.
• Responds to requests for member “wellness checks”, provides clinical support and intervention to all members of the Terwilliger Plaza Community as needed during clinic hours.
• Actively participated in assessment, development of care plans, monitoring and changes in conditions of all In Home Clients
• Assists in training and education of new staff and orientation of new members
• Reports to the Director of Ally In-Home Services
• Acts as Assigned Designee in the absence of the Director and is able to manage administrative functions as necessary
• Compliance and participation on all RN requirements under the OHA and OSBN licensing agencies
• Maintains timely and accurate documentation in client records
• Practices and adheres to facility policies, protocols ensuring good safety, staff development, interdisciplinary collaboration and public relations
