Mary's Woods is looking for a Resident Care Coordinator who collaborative and dedicated to join our Health Services team. The Resident Care Coordinator supports their assigned neighborhood by facilitating communication and coordination with residents, families, other Mary’s Woods Departments, and outside providers. This position ensures team members have the resources to provide excellent care and services, which includes but is not limited to medical supply ordering and management, office supply management, work order submissions, record keeping, and chart audits. This position also serves as back-up to the Health Services Staffing Coordinators and works in a direct care capacity as needed to ensure resident needs are met.

Below are some of the core responsibilities, experience, and skills needed to be successful:
- Facilitates day-to-day coordination of resident care.
- Coordinates outside providers and assists with scheduling resident appointments, including arranging transportation as needed.
- Schedules care conferences, including communication with all invited attendees and participates in care conferences both as a content contributor and as administrative support.
- Conducts periodic audits, including but not limited to residents’ charts, Medication Administration Record (MAR), Treatments Administration Record (TAR).
- Works collaboratively with Nurse Manager on resident, family, and team member concerns.
- Assists with coordination and facilitation of admission and discharge orders.
- Assists with the resident move-in process including but not limited to ensuring homes are ready for the resident, resident orientation, and follow-up on resident satisfaction.
- Responds promptly to emails, voice mail messages and faxes to promote effective departmental communications.
- Manages and orders supplies for staff and residents.
- Understands, complies with, and promotes all rules regarding resident rights and community-based care.
- Promotes positive relationships with residents, visitors, and regulators, including but not limited to presenting a professional appearance and attitude.
- Recognizes opportunities to make improvements within the department and takes appropriate action.
- Collaborates with various healthcare professionals and vendors to ensure residents receive quality care.
- Works as a Caregiver or Medication Technician as needed to meet resident care needs.
- Fills in for the Staffing Coordinator role as needed.
- Able to work as a team with employees at all levels.
- Supports and follow all safety and infection control practices including attending safety meetings.
- Strictly adheres to Health Insurance Portability and Accountability Act (HIPAA) privacy requirements.
Education and Experience Required:
- Minimum one year of experience working as a Resident Care Assistant and/or Certified Nursing Assistant.
- Minimum three years of experience in a healthcare community
- Education equivalent to the completion of a high school diploma or

Schedule: This is a full-time, hourly position typically scheduled to work Monday to Friday from 8:30 am to 5:00 pm. The work schedule requires flexibility to accommodate departmental needs.
The starting wage depends on experience, certification and education.
