Who are we looking for?

We’re seeking an experienced Registered Nurse to serve as our MDS Coordinator & Care Manager within our 7-bed boutique Skilled Nursing Facility—an intimate setting where personalized attention and exceptional quality truly define the resident experience. In this role, you won’t just oversee assessments and documentation; you’ll help shape a highly individualized rehabilitation journey for each resident, ensuring care is thoughtful, responsive, and centered on their goals, preferences, and dignity.

 

You’ll lead the MDS and care planning process from start to finish, partnering closely with our Interdisciplinary Team to ensure assessments are accurate, timely, and reflective of each resident’s true clinical picture. From coordinating ARDs to facilitating care conferences, you’ll play a central role in keeping residents, families, and team members aligned and informed. Your attention to detail will directly support compliance, quality outcomes, and reimbursement integrity.

Beyond documentation, you’ll serve as a clinical resource and leader within our close-knit SNF team. You’ll review and strengthen documentation practices, participate in Medicare and Triple Check reviews, support QAPI initiatives, and collaborate with therapy, pharmacy, and medical records to ensure seamless care coordination. In our intimate setting, your voice and expertise will truly make a difference.

What are the requirements?

  • Current RN license in Oregon
  • Minimum 1 year experience with MDS coordination, PDPM, OBRA, RAI processes, and CMS regulations
  • Minimum 1 year Skilled nursing or long-term care experience required
  • Strong knowledge of clinical documentation, reimbursement systems, and regulatory compliance
  • Excellent organizational, communication, and problem-solving skills
