At Friendsview, we believe that transitions matter. As our LPN Admission Nurse, you'll play a key role in helping older adults and their families navigate one of life's most important transitions with confidence, compassion, and dignity.

This isn't a traditional floor nursing role. You'll be the clinical coordinator behind seamless admissions, working closely with residents, families, providers, and interdisciplinary teams to ensure every new resident receives a safe, organized, and welcoming start in our Residential Care community.

What You'll Do

As the primary clinical contact for admissions, you'll:

  • Coordinate new admissions, readmissions, and short-stay resident transitions.
  • Review clinical referrals, medical records, and admission documentation.
  • Complete nursing assessments and support person-centered care planning.
  • Conduct medication and treatment order reviews and assist with medication reconciliation.
  • Partner with residents, families, physicians, hospitals, therapists, and community providers.
  • Ensure all admission documentation is accurate, complete, and compliant.
  • Help residents and families feel informed, supported, and welcomed.
  • Collaborate with nursing, caregiving, therapy, and administrative teams to create smooth transitions into care.
  • Support continuity of care for Friendsview Life Plan members utilizing short-stay or recovery services.
  • Follow up with new residents to ensure a positive admission experience.

What We're Looking For

Required Qualifications

  • Current Oregon LPN license in good standing.
  • Current CPR/BLS certification (or ability to obtain)
  • Strong clinical assessment and documentation skills.
  • Excellent communication and organizational abilities.
  • Ability to manage multiple priorities while maintaining attention to detail.
  • Commitment to person-centered care and exceptional customer service.

Preferred Qualifications

  • Experience in assisted living, residential care, skilled nursing, rehabilitation, home health, hospice, or senior living.
  • Experience with admissions, transitions of care, discharge planning, or medication reconciliation.
  • Familiarity with electronic health records (EHRs).
  • Knowledge of Oregon Residential Care Facility regulations.
